TRC Conversation: Care Coordination for Blood Pressure Control

What Happened?

An 87-year-old woman was over 20 days late to fill Diovan® (valsartan), a blood pressure medication at her local pharmacy.

Clinical Gap in Care

High blood pressure puts excessive strain on the cardiovascular system, causing damage to the heart and blood vessels. Without regular treatment, patients are at increased risk for heart attack or stroke.

Specialist Pharmacist Intervention

A specialist pharmacist from the Express Scripts’ Cardiovascular Therapeutic Resource Center® detected the patient’s adherence gap and made an outreach. The pharmacist spoke with the patient’s daughter, who was the primary caretaker.

Case Study Controlling Blood Pressure

This is a recording of an actual call between a patient’s daughter and an Express Scripts specialist pharmacist. The daughter’s voice has been altered to protect the patient’s privacy.

The specialist pharmacist explained the importance of better adherence to blood pressure medication to reduce the risk of hospitalization or death from heart attack or stroke. By improving care coordination among the patient, her caregiver and pharmacy, the specialist pharmacist set the patient on the track to healthier outcomes.

Why It Matters

Specialist pharmacists are highly trained in behavioral health communications, such as motivational interviewing, to help patients overcome issues related to adherence when they are ambivalent to changing their behavior. Patients are much more likely to become adherent when they make the decision themselves and understand the importance of their medication in a nonprescriptive fashion that supports their self-efficacy.

A strong roster of specialist pharmacists focusing on patients with needs in a specific area, such as the Cardiovascular TRC, is one of the unique ways Express Scripts helps patients stay on the right treatment and avoid unnecessary, harmful, adverse events.
