What Happened?

A 42-year-old athletic coach with diabetes was not taking metformin to manage his blood sugar levels.

Clinical Gap in Care

Metformin is a key medication for a patient with Type 2 diabetes. Metformin is the first medication suggested in the American Diabetes Association recommendations to be used in combination with diet and exercise. By not taking metformin, patients reduce control of blood glucose which can lead to eye, kidney and nerve damage and possible cardiovascular issues.

Specialist Pharmacist Intervention

Express Scripts’ solution ScreenRx® combines early detection and tailored interventions to address adherence challenges.

With this patient participating in the ScreenRx program, a specialist pharmacist from the Diabetes Therapeutic Resource Center® detected the patient’s nonadherence and made an outreach. The patient believed he could control his diabetes with diet and exercise alone.

The specialist pharmacist was able to explain how the medication could work along with his lifestyle changes to help him improve his blood sugar level and attain his goals sooner. Once he reached his goal, he could work with his prescriber to potentially stop the metformin therapy.

Why It Matters

Specialist pharmacists are highly trained in behavioral health communication skills, such as motivational interviewing, to help patients overcome issues related to adherence when they are ambivalent to changing their behavior. Patients are much more likely to become adherent when they make the decision themselves and understand the importance of their medication in a nonprescriptive fashion that supports their self-efficacy.

After discussion with the specialist pharmacist, the patient committed to take metformin along with lifestyle changes.
